Purchaser Information

  • If you are the purchaser, please list your name and address in this area.
  • We will debit your bank account at the first of every month.

Metropass User Information

  • Complete this section only if you want the pass mailed to a different address. If you are buying a pass for another person, please enter that person’s name and address. If you are using a Post Office Box please enter the PO Box number. Your permanent address is still required in the first section)

Information Required for Debit of Bank Account

  • Chequing Account
    Please remember that you must include a cheque clearly marked “void” so we can debit your chequing account. Please note that line of credit or credit card cheques are not accepted.
  • Savings Account
    If you do not have a chequing account, you can use your savings account. Please take your application to the bank and have your banking information filled out and officially stamped by a bank representative.

Type of Pass and Quantity

  • Please enter the type and quantity of Metropasses you need per month in the spaces provided.  Also, enter the quantity of Downtown Express Fare Stickers you need per month.
