Note: All of the attached forms provided below can be filled out and then printed. You will not be able to save or submit any of the attached forms below electronically.
Address change form
To change your address you can notify us either by phone, fax, e-mail, mail or in person. When submitting a change, please include your name, customer number, your old and new address along with the postal code and your new telephone number. Please sign and date your request.
Note: Your change of address information must be received by the MDP Office by the 5th day of the month prior to the effective month you would like it effective by, e.g. April 5th for a May pass.

Change your banking information
To change your banking information you need to send us a fully encoded VOID cheque either by fax, mail or in person no later than the 25th of the month. NO BANK CHANGES WILL BE ACCEPTED BY PHONE OR E-MAIL.
When submitting your change, please include your name, address, telephone number, your customer number, your old banking information and a copy of your fully encoded VOID cheque. Please sign and date your request.

Change your Metropass MDP type
To change the type of pass you receive, you must notify MDP by phone, fax, e-mail, mail or in person.
Please note, depending on the type of change you are requesting, a price adjustment may apply. The request to make this change must be received by the 5th day of the month. If you have already received your pass and want to exchange it for the appropriate type, please visit the MDP Office.

Paying for an outstanding MDP account balance
If you have received an invoice because previous payment has not been received by the MDP office, you must pay that invoice plus pay for the next month’s Metropass by the due date on the invoice. Failure to do so will impede the mailing out of your next month’s pass.
If you are paying via fax, we accept either VISA or MasterCard. Please fill out the back of the invoice completely and fax it to 416-338-0210. You can also obtain a copy of this payment form by clicking on the link below, completing it, print and then fax it to 416-338-0210.
If you send your payment by mail, the only acceptable forms of payment are Certified Cheque or Money Order. Please remit payment along with top portion of your invoice. If you are paying in person, we accept Cash, Money order, Certified Cheque, Visa, MasterCard, and Debit.
Cancellation or quantity change
To increase, decrease, or cancel your subscription, you must notify us either by phone, fax, e-mail, mail or in person.
When submitting a change, please include your name, address, telephone number, and customer number if you have it. Please make sure to sign the request as well. Please note that if you are canceling or decreasing your subscription, cancellation fees may apply.
Please refer to the Terms and Conditions of the contract. The request to increase, decrease, or cancel your subscription must be received by the 5th day of the previous month you would like it effective by (April 5th for May change). To make this change by phone, call our office at 416-397-8827. You will need to verify personal information before we can make the change.
Cancellation fees
Cancellation fees are due when the purchaser cancels their plan prior to the completion of any 12-month period. See terms and conditions #10. Cancellation fees are calculated by determining the difference between the regular cost of the monthly Metropass and the reduced monthly cost of the Metropass, on the Metropass Discount Plan, multiplied by the number of passes used during the current 12 month period.
