How to apply for the Metropass Discount Plan?

Step One

Step Two

Get a Metropass Discount Plan application form.

• You can pick up your Metropass Discount Plan application form from
  • Any subway station collector booth
  • The Metropass Discount Plan Office

OR

• You can also access the MDP Application Form online.

Step Three

Fill out the Metropass Discount Plan application form completely. If you are using our online Metropass Discount Plan application form, please fill out all the sections and then print it out. Note you will not be able to submit the online form electronically.

Step Four

We must receive your completed MDP application by the 5th day of the month that precedes the month you wish to receive your first pass, e.g. April 5th for a May pass.

You can submit the completed Metropass Discount Plan application form along with a fully encoded VOID cheque by fax at 416-338-0210 or by mail to:
